Who this is for
Employers with Benefits, and Feature Admins with 1095 administration access, can confirm employee information on the 1095-B List.
To access the 1095-B List:
- In the top navigation, select 1094/1095.
- In the secondary navigation, select List.
If 1094-B/1095-B configuration is not finished, the page shows: To access this page, finish configuring your 1094/1095 subgroups. Complete configuration first, then return to List.
When the list is ready, the 1095-B Employees page shows each employee's status (Included or Not Included), name, and monthly coverage marks, plus counts for Enabled Forms and Disabled Forms.
To filter the list:
- Open the -Filter Employees- dropdown and choose Included or Excluded (matches Not Included status), or leave the default to show everyone.
- Optionally type a name in Search Employees.
- Select Reset to clear the filter and search.
To edit an employee's 1095-B:
- On the employee's row, hover the actions gear and select Edit.
- In Edit 1095-B form for [employee name], update Part I (Responsible Individual), Part II (Employer-Sponsored Coverage), Part III (Issuer or Other Coverage Provider), and Part IV (Covered Individuals) as needed.
- Select Save Changes (or Cancel to close without saving).
To include or exclude an employee:
- On the employee's row, hover the actions gear.
- If status is Included, select Don't include to set status to Not Included.
- If status is Not Included, select Include to set status to Included.
To add employees who are missing from the list:
- On 1095-B Employees, select Add Employees.
- In Add New Employees, choose Add An Individual Employee or Import Employees Using 1095-B Spreadsheet.
- For an individual, complete Add employee, including 1094 Subgroup, then select Save Changes.
- For a spreadsheet import, follow the Import modal steps (Download template, fill it in, upload, then Import). See 1095-B Census Instructions for the template fields.
Manually added employees show a Delete action. Employees who auto-populated on the list do not; if a 1095-B should not be filed for one of those employees, use Don't include instead.
