After you create a Long Term Disability plan, set premiums on the Premiums tab. When creating the plan shell, select Long term disability or Long term disability (2) as the Benefit Type. To create the plan shell first, follow How To Create Plans in BerniePortal.
Who this is for
Employers with Benefits enabled, Brokers, Agencies, General Agencies, Benefit Providers, and Service Team Members with write access can create Long Term Disability plans and set premiums on plans they can edit.
To set Long Term Disability premiums:
- Create a Long Term Disability plan, or open an existing Long Term Disability plan from Benefits → Plans. After you save a new plan, BerniePortal opens the Premiums tab. For an existing plan, open it and select Premiums.
- Under Purchase Dependency:, complete coverage determination, salary-change timing, premium calculation strategy, and volume rounding.
- Complete Premiums: (age-banded monthly rates).
- Complete AGE CALCULATIONS.
- Complete purchase amount or purchase minimums/maximums for the coverage path you selected, and EMPLOYER CONTRIBUTION: when it appears.
- Complete EOI:, including uploading EOI document required (PDF) when that field appears.
- Select Save Changes.
Purchase Dependency:
Complete these settings:
- How is the amount of coverage determined? — Fixed percentage of salary or Employee chooses
- When will salary change affect plan premiums? — At salary change effective date, First of the month following salary change, or At plan anniversary date
- How is proration calculated? — Based on exact number of days or Based on 30 days (appears when salary-change timing is At salary change effective date)
- How are premiums calculated? — Per $100 of monthly benefit or Per $100 of monthly covered payroll (Per $100 of monthly covered payroll is available only for Fixed percentage of salary)
- Should “Volume” be rounded? — No or Yes (appears for Fixed percentage of salary; hidden for Employee chooses)
Long Term Disability premium strategy uses monthly benefit / monthly covered payroll. Do not enter Short Term Disability weekly-per-$10 strategy options here.
Premiums:
Enter age-banded rates. These values are entered per month for the How are premiums calculated? strategy you selected (per $100 of monthly benefit or monthly covered payroll):
- For Employee chooses, enter Purchase increment (costs per)
- For each Age Range row, enter Premiums. Use Add new age range as needed
- Optionally enable Gender Premiums (Male / Female columns)
- Tobacco Premiums appears when the plan is set so tobacco use affects rates; when enabled, enter Non Tobacco and Tobacco values
For Long term disability (2), also complete Employee Benefit Reduction Levels (age ranges with percentage reduction).
AGE CALCULATIONS
For Employee, select Actual age or Current year - Birth year, then select How should a new hire's age be calculated? — Age at effective date or Age at most recent renewal date.
PURCHASE AMOUNT / MAXIMUMS:
When How is the amount of coverage determined? is Fixed percentage of salary, enter:
- Benefit percentage of salary
- Maximum monthly benefit
PURCHASE MINIMUMS: and PURCHASE MAXIMUMS:
When How is the amount of coverage determined? is Employee chooses, enter:
- PURCHASE MINIMUMS: — Minimum amount
- PURCHASE MAXIMUMS: — Benefit percentage of salary and Maximum monthly benefit
EMPLOYER CONTRIBUTION:
When Fixed percentage of salary is selected, set Percentage paid by employer (0–100). This section does not appear for Employee chooses.
EOI:
Answer the EOI question with No or Yes:
- Fixed percentage of salary — Require EOI if employee previously waived coverage
- Employee chooses — Require EOI if employee previously waived coverage or is requesting additional coverage over amount previously approved
If Yes, upload EOI document required (PDF) when the file field appears. For some carrier-integrated employers, BerniePortal provides EOI documents automatically during enrollment instead of showing the upload field.
