How to Create a Health Plan

After you create a Health plan, set monthly premiums on the Premiums tab. To create the plan shell first, follow How To Create Plans in BerniePortal.

Who this is for

Employers with Benefits enabled, Brokers, Agencies, General Agencies, Benefit Providers, and Service Team Members with write access can set Health plan premiums on plans they can edit.

To set Health plan premiums:
  1. Create a Health plan, or open an existing Health plan from BenefitsPlans. After you save a new plan, BerniePortal opens the Premiums tab. For an existing plan, open it and select Premiums.
  2. In HEALTH PREMIUM TYPE, select one of these options:
    • Composite
    • Age Banded Health (one table)
    • Age Banded Health (three table)
    • Age Banded Multiperson
  3. Complete the premium fields for the type you selected. Premiums are entered per month.
  4. Select Save Changes.
Composite

Under Coverage Tiers:, enter the Monthly Premium for each coverage tier that applies to the plan. New Health plans default to Composite.

Age Banded Health (one table)

Complete these sections:
  • AGE CALCULATIONS for Employee, Spouse, and Child
  • Premiums: age ranges and monthly premiums (use Add new age range as needed)
  • Limit on Cost of Dependents — answer Is there a limit on the number of dependents for whom an employee can be charged? If you select Yes, also set What is the limit? and Up to and including what age do dependents count towards that limit?
Limit on Cost of Dependents appears only for Age Banded Health (one table).

Age Banded Health (three table)

Complete these sections:
  • AGE CALCULATIONS for Employee, Spouse, and Child
  • Separate premium tables for employee, spouse, and child age ranges (use Add new age range as needed)
Age Banded Multiperson

Complete these sections:
  • AGE CALCULATIONS
  • Premiums: age ranges across coverage tiers (use Add new age range as needed)
