Configuring your 1094-C/1095-C forms

Once you have finished configuring your subgroups, you should navigate HERE to begin configuring your subgroups. Click on your 1094 subgroups to configure them. In the image below, we will click “All Employees” to begin our configuration:



IRS instructions on how to configure these steps can be found HERE. Below is a summary of these instructions, if you need more detailed instructions, please refer to the IRS link in the previous sentence:

  1. Part 1. Applicable Large Employer Member (ALE)
    1. Lines 1-8: Fill in the appropriate information.
      1. For Line 2, if the employer has multiple EINs, they should create multiple 1094 subgroups. (Return to “Organizing your 1094/1095 Forms”)
      2. For Line 7, the HR person you are working with is the appropriate person.
    2. Lines 9-16: Leave blank unless the employer is a government entity. If it is a government entity, then fill them out as appropriate.
    3. Line 17: Omitted because it’s not necessary to fill it in on the forms.
    4. Line 18: Will be auto-populated by BerniePortal after 1094-C and 1095-C subgroups have been configured.
  2. Part II. Applicable Large Employer Member (ALE).
    1. Line 19: YES if this particular 1094 is the authoritative transmittal. NO if this particular 1094 is not the authoritative transmittal.
    2. Line 20: Leave blank, will be auto-populated by BerniePortal after 1094-C and 1095-C subgroups have been configured.
    3. Line 21: NO if this ALE is not part of an aggregated ALE group. YES if it is.
    4. (22) Certifications of Eligibility: Select all that apply for the employer, either A, D, both A and D, or neither.
  3. Part III. ALE Member Information - Monthly (will only populate if question 19 is YES
    1. Column A: Indicate YES or NO in "All 12 Months" row, OR Indicate YES or NO for each month.
    2. Columns B and C: These will be auto-populated by BerniePortal after 1094-C and 1095-C subgroups have been configured. You can manually overwrite these numbers if desired.
    3. Column D: Check the box if you selected YES on Line 21.
    4. Column E: This column was previously designated for 4980H transition relief for 2016. 4980H transition relief is not relevant for this tax year. Therefore this column requires NO action.
  4. If you selected YES on Line 21, type the business names and EINs in the "Other Members of Aggregated ALE Groups" section with the text fields provided. This will need to be replicated in each 1094 subgroup where you have selected YES on Line 21.

Repeat these steps for each 1094-C subgroup that the employer has until all have been completed. Then you can move to the 1095-C subgroup configuration:



Click the subgroup name of each 1095-C to indicate the default values for Lines 14-16 of the 1095-C forms. The 1095-C subgroups are based on your Benefits Subgroups in BerniePortal.

  1. Default Plan Start Month: Indicates the start month of your employer's coverage.
  2. Line 14 (Default Employer Offer of Coverage Code) - Indicates what kind of coverage the employee offered.
  3. Line 15 (Default Employee Share of Lowest Cost Monthly Premium, for Self-Only Minimum Value Coverage) - Enter the monthly employee cost of the lowest cost plan for employee only coverage for each month during the year. If the amount is the same for all 12 months, check the appropriate box, otherwise enter the amount for each month.
    1. A line 15 value is required when the code in Line 14 is 1B, 1C 1D, 1E, 1J, 1K 1L, 1M, 1N, 1O, 1P, 1Q, 1T, or 1U.
  4. Line 16 (Applicable Section 4980H Affordability Safe Harbor Code) - Indicate the employer's default safe harbor affordability codes.
    1. BerniePortal will not display Codes 2A, 2B, 2C, or 2D, as these codes are not safe harbor codes. 
    2. BerniePortal will apply codes 2A, 2B, 2C, or 2D automatically to the applicable employees and the corresponding months for each code.
  5. All part-time employees - Check this box if this subgroup contains only part-time employees.
  6. Self-insured - Check this box if the coverage offered to employees in this benefits subgroup was self-insured coverage
    1. Be sure to answer this correctly, as the answer to this question will generate the 1095-C forms correctly for the employer.
Select "Save Changes", and then complete the steps above for each 1095-C subgroup listed.

Once you have completed this step, the employee list will generate! Your next step will be to confirm employee information in the 1095-C employee list.
