Configuring your 1094-B/1095-B forms

Once you have finished configuring your subgroups, you should navigate HERE to begin configuring your subgroups. Click on your 1094 subgroups to configure them. In the image below, we will click “All Employees” to begin our configuration:

  1. Part I - Responsible Individual
    1. Lines 1-8: Fill in the appropriate information
      1. For Line 3, the HR Administrator you are working with is who you should enter.
    2. Line 9 will be filled in when you are finished configuring the 1094-B/1095-B subgroups.



Once you have finished the 1094-B subgroup configuration, you can move to the 1095-B configuration:



Click the subgroup name of each 1095-B to indicate the default start month and origin of the health coverage. The 1095-B subgroups are based on your Benefits Subgroups in BerniePortal.

  1. Default Plan Start Month: Indicates the start month of the employer's health coverage.
  2. Origin of the Health Coverage: If you have questions about which code applies to the employer, you should work with a tax professional.
Select "Save Changes", and then complete the steps above for each 1095-B subgroup listed.

Once you have completed this step, the employee list will generate! Your next step will be to confirm employee information in the 1095-B employee list.
