Configuring your 1094-B/1095-B forms

After you organize your 1094 subgroups, configure each 1094-B form and set defaults for each 1095-B subgroup. For IRS field definitions, use the Instructions for Forms 1094-B and 1095-B.

Who can configure these forms

Employers with Benefits enabled, and Feature Admins who can administer 1095.

Before you begin

Finish Organizing your 1094/1095 Forms so every employee is assigned to a 1094 subgroup, and confirm you selected File 1094-B/1095-B for the tax year. If you file C forms instead, see Configuring your 1094-C/1095-C forms.

Configure a 1094-B form
  1. Go to top nav 1094/1095, then secondary nav Subgroups.
  2. In the left sidebar, select 1094-B to open the 1094-B Subgroups table.
  3. Open a subgroup by clicking its name in the Subgroup Name column, or use ActionsEdit.
  4. On the Form 1094-B screen, complete required fields (1) Filer's Name, (2) Employer identification number (EIN), (3) Name of person to contact, (4) Contact telephone number, (5) Street address (including room or suite no.), (6) City or town, (7) State or province, and (8) Country and ZIP or foreign postal code.
  5. (9) Total number of Forms 1095-B submitted with this transmittal is read-only and filled from attached 1095-B forms.
  6. Select Save configuration. Repeat for each 1094-B subgroup.
Configure 1095-B subgroup defaults
  1. From 1094/1095Subgroups, select 1095-B in the left sidebar to open 1095-B Subgroups.
  2. Click a subgroup name in the Subgroup Name column. The Default code dialog opens. These subgroups come from your Benefits subgroups.
  3. Confirm Benefit subgroup name and set Default Plan Start Month (coverage start month as 0112).
  4. Select (8) Origin of the Health Coverage. Choose the letter that matches your coverage: A. Small Business Health Operations Program (SHOP), B. Employer-sponsored coverage, except for an individual coverage HRA, C. Government-sponsored program, D. Individual market insurance, E. Multiemployer plan, F. Other designated minimum essential coverage, or G. Employer-sponsored coverage that is an individual coverage HRA. If you are unsure which code applies, consult a tax professional or the IRS instructions linked above.
  5. Select Save Changes. Repeat for each 1095-B subgroup.
