Forms - Benefits

If your broker has added carrier forms for your employer to communicate eligibility, they will be available for import in the "Forms" area of the "Benefits" feature.

To import available forms:
  1. Select the "Benefits" tab in the top navigation menu. 
  2. Select "Forms" from the secondary navigation menu. 

  4. If you'd like to import a new form, select the "Import Form" button in the top right corner. 
  5. Select the form from the listing, then select "Add."

  7. The form will now be listed in the Forms library, and can be added to your employer's plans. Learn more here.
