Line 15 value is missing from the employee form (1095-C Error Only)
When the IRS reports that Line 15 is missing on a 1095-C employee form, enter the employee contribution amount for every month where Line 14 uses one of these codes: 1B, 1C, 1D, 1E, 1J, 1K, 1L, 1M, 1N, 1O, 1P, 1Q, 1T, 1U.
To update Line 15:
- In the top navigation, select 1094/1095.
- Select the List secondary tab.
- On the 1095-C Employees list, find the employee whose Line 15 amount needs to be updated.
- In the Actions column, open the menu and select Edit.
- In the Edit 1095-C Form modal, under Part II — Employer Offer and Coverage, enter the correct amount in the Line 15 row for each month that requires it. Use the All 12 Months column when the amount applies for the full year.
- Select the Corrected form checkbox.
- Select Save Changes.
