Employee name and/or SSN does not match what's in the IRS database (1095-C Error Only)

This 1095-C e-filing error means the employee name and/or Social Security Number on the form in BerniePortal does not match what the IRS has on file.

If the name and SSN in BerniePortal are already correct, contact IRS E-Services to confirm what the IRS has on file for the employee.

Who can correct this

Employers (with Benefits), and Feature Admins who can administer 1095.

To correct the employee name and/or SSN on the 1095-C form:
  1. In the top navigation, click 1094/1095.
  2. In the secondary navigation, click List.
  3. On the 1095-C Employees list, find the employee. You can use Search Employees.
  4. In the Actions column for that row, open the menu and click Edit.
  5. In Part I — Employee Information, update First Name, Middle Name, Last Name, and/or Social Security Number (SSN) as needed.
  6. Select Corrected form.
  7. Click Save Changes.
Editing the 1095-C form does not change the employee profile. To update the profile for future filings when the employee name on the list is a link:
  1. On the 1095-C Employees list, click the employee's name.
  2. In the Personal Info section, click the pencil icon.
  3. Update First Name, Middle Initial, Last Name, and/or Social Security Number as needed.
  4. Click SAVE.
