A covered individual or dependent's monthly coverage information is not fully filled-out

This error is triggered when the months of coverage for a covered individual or dependent(s) in BerniePortal is not filled-out for at least one covered individual or dependent with an employee. 

NOTE: This error will only impact self-insured employers. If you are not a self-insured employer, you will need to review your 1095-C subgroup default codes and confirm "self-insured" is not selected. 

To correct these errors, you will need to do the following:
  1. Log into the employer account in BerniePortal.
  2. Click on "1094C/1095C" at the top of the page.
  3. Click "List".
  4. Find the employee whose name and/or SSN needs to be updated.
  5. Hover over the action wheel and click "Edit":

  6. Scroll down to "Part III. Covered Individuals" and update either Section D (Covered All 12 Months) or Section E (Months of Coverage):

  7. Scroll down to the bottom of the page and select "Corrected Form".
  8. Push "Save Changes".
