A covered individual or dependent's monthly coverage information is not fully filled-out

When the IRS reports that a covered individual or dependent's monthly coverage is incomplete, complete coverage for each covered individual listed on the employee form. For every covered individual with a name, select either (D) Covered All 12 Months or at least one month under (E) Months of Coverage (JanDec).

This applies when self-insured coverage information is included on the form. Employers and Feature Admins with 1095 access can make this correction on the employee list. If the form was already e-filed, also mark it as corrected so the IRS accepts the update — see Marking an Employee Record as "Corrected" in BerniePortal.

To complete monthly coverage (1095-C):
  1. In the top navigation, select 1094/1095.
  2. Select the List secondary tab.
  3. On the 1095-C Employees list, find the employee whose covered individual coverage needs to be updated.
  4. In the Actions column, open the menu and select Edit.
  5. In the Edit 1095-C Form modal, go to Part III — Covered Individuals. Confirm Include self-insured coverage information is selected so the covered individuals table is shown.
  6. For each covered individual who needs coverage months, select (D) Covered All 12 Months, or select the applicable months under (E) Months of Coverage (Jan, Feb, Mar, Apr, May, Jun, Jul, Aug, Sep, Oct, Nov, Dec). Selecting (D) Covered All 12 Months clears the individual month checkboxes, and selecting any month clears (D) Covered All 12 Months.
  7. Select the Corrected form checkbox.
  8. Select Save Changes.
If your List page shows 1095-B Employees instead, open Edit for the employee, then complete coverage under Part IV. Covered Individuals using (d) Covered all 12 months or (e) Months of Coverage, select Corrected Form, and select Save Changes.

If your organization is not self-insured and this error still appears, review your 1095 subgroup settings and clear Self-insured if it was selected by mistake.
