A covered individual or dependent name and/or SSN does not match what's in the IRS database

This 1095-C e-filing error means a covered individual's name and/or SSN on the form does not match what the IRS has on file. Covered individual rows appear only when the form includes self-insured coverage.

Employers and Feature Admins who can administer 1095 can correct this information.

If BerniePortal already matches the legal name and SSN

Contact IRS E-Services at 866-255-0654 to confirm the name and SSN the IRS has on file.

If the error is for the employee (not a covered individual)


To correct a covered individual on the 1095-C
  1. Open 1094/1095, then select List.
  2. Find the employee whose covered individual information needs to change.
  3. In that employee's row, open the actions menu and select Edit to open Edit 1095-C Form.
  4. Under Part III — Covered Individuals, confirm Include self-insured coverage information is selected so the covered individual fields are shown.
  5. In the covered individual row, update First Name, Middle Name, Last Name, and/or (B) SSN as needed. If you clear the SSN, enter the date of birth in (C) DOB (if SSN is not available) using YYYY-MM-DD.
  6. Under Part II — Employer Offer and Coverage, select Corrected form. For why this is required, see Marking an Employee Record as "Corrected" in BerniePortal.
  7. Select Save Changes.
If this employer is not self-insured and you do not expect covered individual rows, open 1094/1095Subgroups, edit the 1095-C subgroup, and confirm Self-insured is not selected.
