A covered individual or dependent name and/or SSN does not match what's in the IRS database

This error is triggered when the name and/or SSN of a covered individual or dependent(s) in BerniePortal is different from the legal name and/or SSN that the IRS has for the covered individual or dependent(s).

NOTE: This error will only impact self-insured employers. If you are not a self-insured employer, you will need to review your 1095-C subgroup default codes and confirm "self-insured" is not selected.

If you believe the name and/or SSN are correct for the covered individual or dependent(s), you will need to contact IRS E-Services at 866-255-0654 to confirm that the name and SSN they have on file for the covered individual or dependent(s) match what's in BerniePortal.

If the error is related to a covered individual's SSN, you can delete the SSN, and the IRS will accept the covered individual's name and date of birth.

To correct these errors, you will need to do the following:
  1. Log into the employer account in BerniePortal.
  2. Click on "1094/1095" at the top of the page.
  3. Click "List".
  4. Find the employee whose name and/or SSN needs to be updated.
  5. Hover over the action wheel and click "Edit":

  6. Scroll down to "Part III. Covered Individuals" and update the fields that need to be edited (First Name, Middle Name, Last Name, and/or Social Security Number):

  7. Scroll down to the bottom of the page and select "Corrected Form".
  8. Push "Save Changes".
