How to Edit 1095-C Forms

An employee's 1095-C form can be edited before forms are generated, if necessary. For example, if you notice an employee's demographics or codes need to be adjusted, the employee's form can be manually edited.

Note: If your employer did not use BerniePortal for the full reporting year, then you will likely need to add codes for the months that your employer was not utilizing BerniePortal.

To edit an employee's form:
  1. Select the “1094C/1095C” tab in the top navigation menu, then select "List" in the secondary navigation menu. 

  2. Hover over the action wheel to the right of the employee’s name and select “Edit” to open the employee's form in edit mode.

  3. Make the necessary changes to the employee's information in "Part I. Employee Information" and/or "Part II: Employee Offer and Coverage" information. 

  4. Note: If the employee’s 1094-C subgroup needs to be changed, change the 1094-C subgroup first and click “Save”. Only after you’ve done that, click “Edit” again to make any other necessary changes.
