Carrier Forms Report

The Carrier Forms report downloads a single PDF package of completed carrier forms for employees in the subgroups you select. Choose one form from Available forms; BerniePortal includes each employee's most recent completed copy of that form. To download forms for one employee instead, see Personnel File Tab - Employee Profile. To import and attach carrier forms to plans, see Forms - Benefits.

To generate:
  1. As an Employer with Benefits (or a Feature Admin with Benefits access), select Reports in the top navigation.
  2. Select Standard.
  3. Select Carrier Forms.
  4. Under Subgroups, select the subgroups to include.
  5. Under Available forms, select the form to include.
  6. In Send Report To, enter or confirm the email address that should receive the report.
  7. Select Generate & send.
BerniePortal emails that address a link to download the report.
