An employee's information in BerniePortal is causing an error
This IRS error means employee information on a 1095-C form does not meet IRS formatting requirements. Common causes are a missing address field, a State value that is not a 2-letter abbreviation, an invalid ZIP Code, or unsupported punctuation in name or address fields.
Employers and Feature Admins with 1095 access can update these fields on the 1095-C Employees list.
To fix employee information formatting on the 1095-C form:
- Go to 1094/1095 in the top navigation, then select List.
- On the 1095-C Employees page, find the employee whose information needs updating. Use Search Employees if needed.
- In the Actions column, hover over the gear icon and select Edit.
- In Part I — Employee Information, update the fields that are missing or incorrectly formatted. Required address fields are Address, City, State, and ZIP Code. Enter State as a 2-letter abbreviation (for example, OH). Enter ZIP Code as five digits, or five digits plus a four-digit extension (for example, 43215 or 43215-1234). Use only letters, numbers, spaces, hyphens, and ampersands in name and address fields.
- Select Corrected form, then select Save Changes.
To update the employee profile for future filings:
- On the 1095-C Employees page, select the employee’s name in the Name column to open their employee profile. Manually added employees do not have a profile link from this list.
- In Contact Info, select the pencil icon, update Street Address, City, State, and/or Zip Code, then select SAVE.
- If a name or SSN also needs updating for future filings, in Personal Info select the pencil icon, update the fields, then select SAVE.
