Adding Employees to the 1095-C List
Use Add Employees on the 1095-C employee list to add someone who was not tracked in BerniePortal for the reporting year, or who was missed during the year.
Who can add employees
Employers, and Feature Admins with 1095 access, can add employees to the 1095-C list.
If you see a message to finish configuring your 1094/1095 subgroups, complete subgroup setup before the list and Add Employees are available.
To add an individual employee
- Select 1094/1095 in the top navigation, then select List in the secondary navigation.
- On the 1095-C Employees page, select Add Employees.
- In the Add New Employees dialog, select Add An Individual Employee.
Complete the employee fields:
- First Name (required)
- Middle Name
- Last Name (required)
- Social Security Number (SSN) (required)
- Address (required)
- City (required)
- State — choose Select State (required)
- Zip Code (required)
- ALE Subgroup — choose Select ALE Subgroup (required)
- Self-insured (optional)
- Select Save.
The employee appears on the List.
To import employees using a spreadsheet
- Select 1094/1095 in the top navigation, then select List in the secondary navigation.
- On the 1095-C Employees page, select Add Employees.
- In the Add New Employees dialog, select Import Employees Using 1095-C Spreadsheet.
- In the Import dialog, select Download to get the employee template.
- Fill in the spreadsheet. For column guidance, see 1095-C Census Instructions.
- Choose the completed file, then select Import.
